Job for MPHW (M) in Office of the CDMO - 25 posts

Applications in the prescribed proforma are invited from eligible candidates for following posts under the Chief District Medical Officer, Cuttack District to be filled on contractual basis with monthly remuneration mentioned against each of the post.
The Candidates must not be below the age of 21 years or above 32 years as on publication of this advertisement. The upper age limit shall be relaxed by 5 years in case of SC & ST. SEBC. Women candidates, and 10 years in case of physically handicapped / Ex- Serviceman candidates. Age relaxation is however applicable in respect of one category only. The ORV Act and Rule will be applicable for these contractual recruitments.

Qualification:
Must have passed +2 Science Examination under Council of Higher Secondary Education. Odisha/ Equivalent and Diploma in Pharmacy courses passed from any of 3 ( three ) Govt. Medical Colleges of the state or from any other private institutions duly recognized by the Pharmacy Council of India / AICTE/ . Selection of candidates shall be made on the basis of marks secured in HSC (excluding 4 Optional) -(20 %), +2 Science (excluding 411' Optional) -(30 %) and Diploma in Pharmacy (50 %) only Must have passed Odiya as a subject in middle school/ HSC/ Odiya in Middle English school conducted by the School and Mass Education Dept.

1) The applicant have to send their application form through Regd.Post/Speed Post/Courier only in the address of “Chief District Medical Officer, Cuttack,At- Zilla Swasthya Bhawan,Old Secretariat Campus, Buxibazar, Cuttack- 753001” on orbefore 20.05.2016.The authority will not be responsible for any postal delay.
(2) The application received in hand or any other media will not be entertained. (3)Candidates of Cuttack district will be given preference for appointment, in case of non availability of Cuttack district, candidates from other districts will be considered.
(4) Selection will be done strictly on the basis of merit in the qualifying examination as required for the posts and no weightage will be given for higher qualification.
(5) Candidates, who are already working in Health Department either on regular or contractual basis, have to apply through proper channel.
(6) The appointment is purely temporary and may be terminated at any time without assigning any reason thereof.
(7) Age limit- The candidate should not be less than 21 years and not more than 32 years as publication of the advertisement and the upper age will be relaxed up to 5 years in case of SC. ST & SEBC candidates and 10 years for Physically handicapped candidates.
(8) The selected candidates will have to submit an undertaking to the effect that they will not claim for any Govt. Post/ Regular appointment in future.
(9) One envelop should contain only one application for any post. Separate application should be submitted for separate posts in separate envelop.
(10) Application should be clearly mentioned the category of posts applied for on the top of the envelop and also in the application form.
(11) The appointment/ Engagement shall be strictly as per ORV Act.
(12) No of vacancies as mentioned above categorically is subject to change, if required as per rule in future.
(13) Reservation for Women / Physically handicapped person / Ex-Service Man and Sports person will be given as per ORV Act.

(14) The candidates are required to enclose the following self attested documents along with the application form :-
  • Copy of HSC / Equivalent examination certificate and Mark sheet.
  • Copy of Diploma in Pharmacy passed certificate, mark sheet, registration certificate from Orissa Pharmacy Council (for Pharmacist candidates only).
  • Copy of GNM / ANM passed certificate, mark sheet, registration certificate from Orissa Nursing Mid Wife Council (for Staff Nurse and MPIIW(F)/ANM candidates only),
  • Copy of DMLT / DMRT passed certificate, mark sheet (for LT and Radiographer candidates only),
  • Odiya passed Certificate in middle school/ HSC/ Odiya in Middle English school conducted
  • by the School and Mass Education Dept.
  • Copy of valid Residential certificate from competent authority,
  • Copy ot Cast Certificate for SC/ ST/ SEBC candidates from the competent authority.
  • Copy of employment registration Certificate / Card from the employment exchange.
  • Additional qualification/ experience certificate if any.
  • Copy of handicapped certificate if any.
  • 2 nos. of self attested recent passport photographs.
  • Two nos. duly stamped (Rs. 5 each) self addressed envelop size 24 X 10 CM.
